Assignment of Identity Cedula Numbers

Requirements

1. Letter of request signed by the father or mother.
2. Original Bolivian Birth Certificate (Cost of Certificate U$ 5.00 to be paid by Money Order or certified check payable to the Embassy of Bolivia).
3. Original and photocopy of identity card and / or passport of the father and mother.
4. Four color photographs 3cm x 3cm, of the face of the person with a white background.
5. For Adults: Affidavit before Public Notary in which he/she states that he/she never obtained his/her bolivian identity card.
