Birth registration of children born abroad from a Bolivian mother or Bolivian father

Present the following documents:

1. Cover Letter addressed to the Embassy of Bolivia requesting the birth registration, signed by the father, mother or both and the contact details must be included: address, telephone and an email.
2. Original or notarized copy of the Canadian Birth Certificate. The Certificate must indicate the names of the parents. (This document remains in the Embassy for its later sending to tSERECI)
3. Translation into Spanish of the Canadian Birth Certificate ( from a Certified Translator)
4. Original and photocopy of the Parent Marriage Certificate, if applicable
5. Presence of the father and / or mother and the minor.
6. Originals of the identity card or passports of the parents (One of the parents must be Bolivian).
7. Identity card or Passport or identity document of two (2) witnesses, who must be of legal age, and must be present at the time of registration
8. Only for registrations of children between 12 to 18 years old, present a Money Order or Draft payable to Embassy of Bolivia for the value of 15.-US $ (American dollars) and two photos of 3 cm x3cm in color with background White.
