Only for Bolivian citizens

Requirements

  1. Note addressed to the Embassy of Bolivia, attention to the Consular Section requesting the Certificate.
  2. Photocopy of the data sheet of the Bolivian passport.
  3. Photocopy of a Bolivian identity card.
  4. Attach ORIGINAL driver’s license (It will be returned along with the Certificate).
  5. If you do not live in Ottawa, attach prepaid Xpress post with tracking number of Canada Post to send your documents.
