Registry of birth for persons over 18 years of age born abroad from Bolivian mother or Bolivian father

Present the following documents:

1. Canadian Birth Certificate: Original or notarized copy The Certificate must indicate the names of the parents. (The Canadian Certificate stays in the Embassy for later sending to SERECI)
2. Translation into Spanish of the Canadian Birth Certificate ( by Certified Translator)
3. Identity document issued abroad or other documentation proving the identity of the person
4. Identity card, or passport or original birth certificate of the Bolivian mother or Bolivian father
5. Identity card, or passport or original birth certificate of the foreign mother or foreign father (if applicable)
6. Identity card or Passport or identity document of two (2) witnesses, who must be of legal age, and must be present at the time of registration
7. Money Order or Draft in the name of Embassy of Bolivia for the value of 20.-US $ (American dollars)
8. Cover Letter with the contact information
